Service Description

Pediatric Care Package for Ages 1-18 (This includes Teen Avatars, Zooby, Funsies, and Toodledoos) We will perform a physical exam while the parent/guardian is present. This will include an eye exam, teeth exam, listening to the heart and lungs, and paying attention to speech and language development. We will update immunizations. Parents/guardians remember Immunizations can protect your kids from serious childhood illnesses, so it's important that your child get them on time. If any of our doctors assess your child's risk for anemia, lead exposure, and tuberculosis then we will order additional test.

Cancellation Policy

Cancellation of an Appointment Appointments are in high demand and your early cancellation will give another patient the opportunity to be seen by a provider. Missed Appointment Policy A “missed appointment” is an occurrence where someone does not show up for an appointment and does not cancel the appointment in advance of the scheduled date and time. If you do not show up for your appointment and you do not cancel the appointment 24 hours in advance, we will record this in the medical record as a "missed appointment". Each time you miss your appointment, you will be notified by notecard and you will be asked to re-schedule.
